This is confusing!  Here's the syntax, as best I can work out::

    {% blocktrans
        [with parm1=expr1 [parm2=expr2 ...]]
        [count parm=expr]
        [asvar varname]
        [trimmed]
    %}
        text
      [{% plural %}text]
    {% endblocktrans %}
